Athlete Category
- Ontario curler winning two (2) Canadian Championships with at least one (1) being a Brier or Scotties or a team winning a World Championship at Men’s or Women’s Worlds.
- Ontario curler winning any three (3) Canadian Championships
- Ontario curler winning five (5) Curling Ontario Provincials
Individual / Builder Category
- The nominee has made a significant impact or contribution in player development (coaching, education, training, etc.)
- Participation (introducing curling to new participants, expansion of club / community involvement / membership / programs that made curling more accessible) has positively impacted the club.
- Community contribution (leveraging curling to deliver significant charitable, economic, social, physical benefits to their community) has been recognized. This may be achieved through organizing events, outreach, or any other activity that had a positive and significant contribution to the club and or community.
- Innovation (ideas, programs, activities that provided economic and / or operational benefit to member organizations) has been demonstrated.
- Volunteering has been demonstrated. The nominee has a long history of volunteering in any of the above areas such as coaching, education, or training. Significant work in planning and executing events etc.
